Vomiting as an only symptom of a fourth ventricle tumor is rare. We present a 63-year-old female patient with intractable vomiting for one month before a definite diagnosis was made. The vomiting subsided after antiemetic agents were given. The pathological evaluation revealed a high-grade ependymoma arising from the fourth ventricle. The topographical location of the tumor associated with the structure of area postrema implies the mechanical compression as an mechanism for the vomiting.
