Tainan City is a typical city with high-density development. This study takes the West Central District of Tainan City as the research scope, the purpose is to fill the gap in the domestic research on the correlation between urban forms and the elderly’s life satisfaction and perceived health. We also examined the applicability of the foreign empirical findings to the discussion of the relationship between the urban environment and the wellbeing of elderly people in Taiwan. In the stage of empirical analysis, through the axial analysis of the space syntax and cluster analysis, the villages in West Central District regions are clustered into two groups: “Traditional neighborhood” and “Modern neighborhood”, and the questionnaire survey was used to verify the existing arguments. According to the empirical analysis, better spatial characteristics do not have positive impacts on residents’ life satisfaction, while the social space formed by narrow alleys and temple buildings becomes an important place for communication in areas lacking green spaces. That is, for the user, the overall quality of the space is more important than the number of facilities and urban open spaces.
