Background: The Health Promoting Lifestyle Profile-II (HPLP-II) scale measures the multidimensional patterns of a healthy lifestyle and has been translated into several languages and widely tested on diverse populations Based on the findings from Teng et al (2010) the 30 item 5-factor structure of the HPLP-IICR different from Walker’s (1997) original English version with one less factor and 22 fewer items in a sample of Taiwanese adults Objective: The purpose of this study was to determine the best-fitting Chinese HPLP-II factor structure among four proposed models Method: The Chinese HPLP-II instrument was administered to 1 392 adults from four geographical regions of Taiwan: Northern Central Southern and Eastern Confirmatory factor analyses were used to explore several structural models and compared to the previously published factor structures Absolute relative and incremental fit indices were used to evaluate the goodness-of-fit of the 52-item HPLP-IIC and the 30-item HPLP-IICR Results: Neither the original six-factor nor revision five-factor Chinese HPLP-IICR provided suitable model fit with acceptable cut-off values for the fit indexes and thus the two models were improved with post hoc model fitting although only the five-factor 30-item instrument achieved acceptable cut-off values for the fit indexes The reliability and validity of the HPLP-IICR five-factor structure model were confirmed although the model required respecification to reach achieve a suitable model fit as measured by CFI (0 91) GFI (0 90 ) SRMR (0 051) and RMSEA (0 055) The five-factor model thus had a better fit with the data than the original six-factor structure model Conclusion: The five-factor 30-item HPLP-IICR has satisfactory psychometric properties and thus is considered suitable for measuring the multidimensional patterns of a healthy lifestyle among adults in a Chinese population In addition the shortened version 30-item HPLP-IICR scale which is less burdensome for subjects to complete than the original scale can be used in clinical practice to evaluate health promoting lifestyles
