Primary adenocarcinoma of the nasopharynx is a rare tumor. Only 2-5% of all nasopharyngeal cancers are adenocarcinoma. Nasopharyngeal poorly differentiated adenocarcinoma is believed to be resistant to radiation, easy metastasis and poor prognosis. In this report we present a 58 year-old man with a primary nasopharyngeal poorly differentiated adenocarcinoma and neck metastasis. The primary tumor and neck masses achieved a complete response after a concurrent chemoradiotherapy. Unfortunately, bilateral lung metastases were noted 6 months after the completion of treatment. The patient died in spite of aggressive chemotherapy.
