Mixed transcortical aphasia resolving from global aphasia manifested in a 68-year-old right-handed woman after a left middle cerebral artery territory total infarct. Although she was unable to speak at will, she could sing songs as long as she was given the name of a song or the initial part of a song was prompted. She could also recite the syllabary of Japanese. This patient's echolalic repetition, unlike a residual left hemispheric function, and the preservation of the ability to sing support the hypothesis that these two abilities reside in the right hemisphere.
