A 41-year-old man suffered from six episodes of acute neurologic deficits from March 1996 through January 1998. The first episode manifested as right ptosis and diplopia; the second as left ptosis and diplopia; the third as right hemiparesis; the fourth as right oculomotor palsy, left hemiparesis, and left hemihypoesthesia; and the fifth and sixth episodes as recurrence of oculomotor; motor and sensory deficits similar to those of the fourth episode. A diagnosis of clinically definite multiple sclerosis (MS) was made. The MS plaques responded excellently to both dexamethasone, 4 mg IV q6h for 14 days, and methylprednisolone, 1000 mg IV qd for 7-10 days, treatment.
